Lost C Card
Replacement Certification Card
Have you lost your YMCA Scuba certification card or do you need to make changes to an existing card? For YMCA Scuba certified divers from the 1970’s, 80’s and early 90’s, the YMCA Scuba program has a more modern certification card. The new card is mono-colored plastic, uses a student pin number instead of a social security number and no longer has a photo on it. You may even have your number of logged dives on your card if you include your current number of dives on the replacement card form. Unfortunately our records only go back to 1972, so in order to receive a replacement card from before 1972, we will need additional information. Please call our office to learn what we will need from you.
Click on the link below to download the form you will need to complete to request a replacement card:

How Long will it take to receive the replacement card once the replacement card form is sent in?
Upon receiving the above information, the Scuba department usually takes one week to verify the certification, process the paperwork, make the new card and mail it. We appreciate your patience!
* For an additional $25, you may receive your C.M.A.S. international certification card. Please see below for more details.
C.M.A.S. International Certification Card
In 1980, YMCA Scuba became a member of the World Underwater Federation (C.M.A.S.), thus allowing our instructors and divers the distinct designation of this international certification. Due to our high standards, YMCA Scuba remains the only Scuba certification organization in the United States to offer the C.M.A.S. international certification card.
C.M.A.S. (Confederation Mondiale Des Activities Subaquatiques) is the only world diving organization. This organization is composed of some 14,000 diving clubs, 82 national federations and 4.5 million divers.
C.M.A.S. international certifications cards are issued by equivalence in 82 countries and at all levels, for both divers and instructors. The certification notes the following benefits:
- Global acceptance and international recognition
- Allowance to dive in countries where authorization is needed
- Ability to rent equipment and refill cylinders abroad
- Ease in joining international diving groups
In order to receive your C.M.A.S. international certification card or instructor certification, please submit the Scuba Diver Certification Card replacement form (see link above) with the CMAS card choice selected and an additional $25. You must have at a minimum an Open Water YMCA Scuba Diver certification and be a U.S. citizen to receive this card.
