Stanford Health and Lifestyle Assessment: A Tool for Understanding Your Current Health Status

It takes only 15 to 20 minutes to complete and provides you with immediate, personal feedback in a Health Education Report. The report includes information on your health status and risks, with action-focused tips and relevant resources to assist you in making healthy choices for a healthy lifestyle.

What is SHALA?


The YMCA has teamed up with Stanford School of Medicine to bring you the Stanford Health and Lifestyle Assessment (SHALA). This assessment tool will provide you with an overview of your current state of health based on factors such as daily activity, dietary intake and general lifestyle.

SHALA is available to YMCA members and the public.  SHALA will provide you with a multiple-page assessment of your health, as well as information to help you improve your health. The tool is great to print out and share with your doctor, personal trainer or YMCA fitness coach as they work with you.

Follow these simple steps to set-up an account to get started:


1. Go to http://stanfordhealthimprovement.org/activate_america
2. Click "Create an account now!"
3. Select “yes” or “no” to the question, “Are you a YMCA member?”
4. Follow the prompts to select your YMCA, if you are a member.  If you are not a YMCA member, you will be prompted to select the nearest zip code to where you live. If you do not know the nearest zip code, give your best guess. 
5. In the fields provided create a username and password, along with a security question.
