CASE MANAGER

Job Description:
The Harlem YMCA is seeking a qualified individual who will provide social work counseling to residents and prepare them for independent living and help in securing permanent housing.
JOB RESPONSIBILITIES:
- Carry full caseload assigned by Clinical Supervisor.
- Maintain case management records, documents, and reports as required.
- Maintain personal contact with residents, developing a trusting relationship in order to monitor physical, emotional and mental need for additional services.
- Coordinate support and home health care as necessary, making referrals to medical, home care, mental health and/or substance abuse services. Provide assistance in obtaining entitlements.
- Formulate needs assessment and implement service plan in conjunction with program team.
- Manage residents' medical and personal emergencies, such as need for hospitalization, sudden medical or mental hygiene needs.
- Assist residents in establishing and/or strengthening significant relationships and reinforce a supportive network.
- Maintain confidential records, logs, releases and charts for each resident.
- Assist with intake and admission procedures.
- Organize and maintain community resources guides for residents and staff, including clothing and food pantries, meal programs, support groups, skills training, job training, GED, permanent housing resources, etc.
- Develop short term task oriented trainings.
- Participate in weekly meetings.
- Participate with residents in recreational trips and other planned activities.
JOB QUALIFICATIONS: - Bachelor's Degree in Social Work or related field.
- Minimum of two years of experience working in health services field.
- Must have excellent computer skills, workshop facilitation skills and ability to build relationships with others.
- Knowledge of clinical social work, substance abuse, homelessness, housing resources and/or HIV/AIDS.
Salary: $40,163 - $50,203
